PLANES DE COBERTURA  MÉDICA

The plan is NOT insurance. The plan provides discounts at certain healthcare providers for medical services. The plan does not make payments directly to the providers of medical services. The plan member is obligated to pay for all healthcare services but will receive a discount from those healthcare providers who have contracted with the discount medical plan organization. This is a discount plan offered and administered by Orange Medical Discount Plan: 18400 NW 75th Place, Suite #110, Miami, FL 33015.

Orange Medical Discount Plan is the discount plan organization.

The program and its administrators have no liability for providing or guaranteeing medical service or the quality of the medical service rendered. This is not insurance nor is it intended to replace insurance. This discount card program contains a 30-day cancellation period. Enrollment Fees ($30.00 to $10.00 depending on the plan being purchased) cover shipping of Provider Directory, Member ID and other necessary documentation (usually 7 to 10 business days).

You may terminate participation in the first 30 days after receipt of ID card and receive a full refund on any fees or dues paid, less the non-refundable processing fee. After the first thirty (30) days, you may cancel participation at any time and if you have pre-paid any membership fees, the prepayment will be refunded on a pro-rata basis for months you have not used. Orange Medical Plan must receive a notification at least (30) business days in advance of the next billing cycle for you not to be charged for the billing cycle. This program and the program administrators have no liability for providing or guaranteeing service or any liability for the quality of the service rendered.
